Beginners 2 Pro, LLC | 2024-2025 Form for Minors (Under age 18)

I, the undersigned, as the parent or legal guardian of the child named below, do at this moment assume all of the risks of the child participating in any or all baseball activities such as clinics, private and group lessons, travel teams, and all special events hosted by Beginners 2 Pro, LLC. I fully consent to my child participating as a Beginners 2 Pro, LL client.
I acknowledge that the Beginners 2 Pro, LLC. program may carry with it the potential for death, serious injury, and personal loss. I understand that the very nature of baseball can be hazardous. The risks may include but are not limited to, those hazards associated or caused by weather conditions, vehicular traffic, travel, playing conditions, equipment, hitting, pitching, fielding, lack of hydration injuries, the practice and play of baseball, the acts of throwing the ball, getting hit with the ball, having the ball hit back at the pitcher, fielding the ball, swinging the bat, running, jumping, stretching, sliding, diving and collisions with other players, volunteers, and stationary objects, all of which can cause severe injury or death to my child and other participants, actions of other people including, but not limited to, participants, volunteers, spectators, and coaches. I am willing to assume these risks on behalf of my child.

I certify that no health-related reasons or problems may hinder my child’s participation in this Beginners 2 Pro, LLC Program. I certify that my child is physically fit and fully capable of participating in the sport of baseball and that my child is healthy and has no physical or mental disabilities or infirmities that would restrict full participation in these activities, except as made known to coaches and officials of the team.
(A)I WAIVE, RELEASE, AND DISCHARGE from any liability, including but not limited to liability arising from the negligence or fault of the entities or persons released, for death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from. THE FOLLOWING ENTITIES OR PERSONS: Beginners 2 Pro, LLC. And their coaches, agents, representatives, or volunteers.

(B) I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from any liabilities or claims made as a result of participation in this Beginners 2 Pro, LL—program, whether caused by negligence or otherwise. The persons or entities are being released from dangerous or defective equipment or property owned, maintained,d or controlled by them or because of their possible liability without fault. I further agree on behalf of myself and my child listed below, that I shall hold harmless and fully indemnify the parties at this moment, released from any claims, damages, breach of contract, or wrongful conduct of the parties at this moment released, costs including attorney fees, and causes of action which may arise from any cause of action made by me or by, through or on behalf of my child, even if the damages, injuries or death are caused in whole or in part by any of the persons or entities at this moment released. PARENT/GUARDIAN WAIVER FOR MINORS (under 18 years old)

On behalf of my child and myself, I do voluntarily elect to accept and solely assume all risks of injury incurred or suffered by my child (a) while practicing or playing as a member of the team so designated, (b) while serving in a non-playing capacity as a team member or observer during practice or play by other teams or by other players on my child’s team, and (c) while on or upon the premises of any of the fields arranged for by the team. I agree to all the terms on page 1 & page 2 of Beginners 2 Pro, LLC—accident waiver.

PHOTO CONSENT:
I agree that Beginners 2 Pro, LLC and their coaches, agents, representatives, or volunteers may take photographs or digital recordings of me as a participant during these practices, games,s, and events and use them in any media for training or promotional purposes. I further consent that my identity may be revealed therein or by description text or commentary. I waive any rights, claims, or interests and understand there will be no financial or other remuneration.
The accident waiver, release of liability,y and image release shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.
I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT, AND I SIGN IT OF MY OWN FREE WILL.
