Beginners 2 Pro, LLC | Form for Adults and Minors (Under age 18)

I the undersigned, as the parent or legal guardian of the child named below, do hereby assume all of the risks of the child participating in any or all baseball or sport activities such as clinics, private and group lessons, travel teams, and all special events hosted by Beginners 2 Pro, LLC. I give my full consent for my child to participate as a client of Beginners 2 Pro, LLC.
I acknowledge that Beginners 2 Pro, LLC, program may carry with it the potential for death, serious injury, and personal loss. I understand that the very nature of baseball and other sports can be hazardous and risky. The risks may include, but are not limited to, those hazards associated or caused by weather conditions, vehicular traffic, travel, playing conditions. equipment, hitting, pitching, fielding, lack of hydration injuries, the practice and play of baseball and other sports, the acts of pitching the ball, getting hit with the ball, having the ball hit back at the pitcher, fielding the ball, swinging the bat, running, jumping, stretching, sliding, diving and collisions with other players, volunteers and stationary objects, all of which can cause serious injury or death to my child and to other participants, actions of other people including, but not limited to, participants volunteers, spectators, and coaches. I am willing to assume these risks on behalf of my child.


I certify that there are no health-related reasons or problems that may hinder my child’s participation in this Beginners2 Pro, LLC Program. I certify that my child is physically fit and is fully capable of participating in the sport of baseballand that my child is healthy and has no physical or mental disabilities or infirmities that would restrict fullparticipation in these activities, except as made known to coaches and officials of the team. 
(A)I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from thenegligence or fault of the entities or persons released, for death, disability, personal injury, property damage, propertytheft, or actions of any kind which may hereafter occur to me including my traveling to and from. THE FOLLOWINGENTITIES OR PERSONS: Beginners 2 Pro, LLC. and/or their coaches, agents, representatives or volunteers. 

(B) I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraphfrom any and all liabilities or claims made as a result of participation in this Beginners 2 Pro, LLC. Program whethercaused by negligence or otherwise. The persons or entities are being released, from dangerous or defectiveequipment or property owned, maintained or controlled by them, or because of their possible liability without fault. Ifurther agree on behalf of myself and my child listed below, that I shall hold harmless and fully indemnify the partieshereby released from any and all claims, damages, breach of contract or wrongful conduct of the parties herebyreleased, costs including attorney fees, and causes of action which may arise from any cause of action made by meor by, through or on behalf of my child, even if the damages, injuries or death are caused in whole or in part by any ofthe persons or entities herby released. Parent’s Initial 
PARENT/GUARDIAN WAIVER FOR MINORS (under 18 years old)

On behalf of my child and myself, I do voluntarily elect to accept and solely assume all risks of injury incurred orsuffered by my child (a) while practicing or playing as a member of the team so designated, (b) while serving in a non-playing capacity as a team member or observer during practice or play by other teams or by other players on mychild’s team, and (c) while on or upon the premises of any and all of the fields arranged for by the team. I agree to allthe terms on page 1 & page 2 of Beginners 2 Pro, LLC. accident waiver. 

PHOTO CONSENT:
I consent and agree that Beginners 2 Pro, LLC and/or their coaches, agents, representatives or volunteers may takephotographs or digital recordings of me as a participant during these practices, games and events and use these inany and all media for training or promotional purposes. I further consent that my identity may be revealed therein or bydescription text or commentary. I waive any rights, claims or interest and I understand that there will be no financial orother remuneration.
The accident waiver, release of liability and image release shall be construed broadly to provide a release and waiverto the maximum extent permissible under applicable law. 
I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THISIS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT ON MY OWN FREE WILL.
